Your Rights & Responsibilities
You have the RIGHT to:
A reasonable response to your requests and needs for treatment of service within the health care provider’s capacity, stated mission and applicable regulations.
Considerate, compassionate, and respectful care that recognizes your personal values and belief system.
Collaboration with your health care provider to make decisions involving your health care, including the right to accept medical care, or to refuse treatment and to be informed of the medical consequences of such refusal.
Information necessary to enable you to make treatment decisions that reflect your wishes and participate in the consideration of ethical decisions that arise in your care.
Be informed of any human experimentation or other research/educational projects affecting your care or treatment.
Personal privacy and confidentiality of information.
Privacy during examinations, to have visitors excused and to be informed why any observer is present, and to grant or refuse another person’s presence.
Have your guardian, next of kin, or legally authorized responsible person exercise certain rights on your behalf.
Expect explanation of any portion of your bill. Where appropriate, our business office staff will assist you in making arrangements for payment of the bill through a payment schedule or assistance program.
You have the RESPONSIBILITY to:
Provide a complete and accurate medical history to the best of your knowledge.
Ask questions and seek clarification about your diagnosis and treatment and participate in decisions involving your care.
Make it known whether a proposed course of treatment is understood, and whether you are willing and able to comply.
Provide information about complications or symptoms.
Be considerate of the rights of other patients and clinical personnel, and to treat them with respect.
